Join our team of competent and compassionate professionals as a Care Coordinator II in the ICC program at Behavioral Health Network!

The master's level Care Coordinator will use their clinical knowledge to work in partnership with the family and current providers to complete the initial diagnostic assessment to determine eligibility for ICC services. The Care Coordinator will facilitate a dynamic team of multidisciplinary professionals providing intensive supports to children with significant behavioral health challenges and their families.

Your experience, knowledge and great skills will support the ICC program with the following:

  • Providing high quality Wraparound services for children/youth and families.
  • Conducting initial and updated comprehensive assessments inclusive of the CANS and CRAFFT (12+ yo); conducts multi-axial diagnostic assessments and Serious Emotional Disturbance determination for referred children and youth.
  • Convenes and facilitates Care Planning Team (CPT) meetings including creation and reviews of the Individualized Care Plan (ICP) and updates the care plan to reflect the changing needs of the child/youth and family.
  • Working directly and maintains regular and consistent communication with the youth, family, and team to identify strengths and needs of the youth and family, and strategies for meeting their needs.
  • Working directly with and fosters youth and family participation in the development, understanding, and implementation of the ICP goals and interventions.
  • Preparing, monitoring, and modifying the ICP in concert with the Care Planning Team.
  • Identifying and actively assisting the youth and family to obtain available medically necessary services.
  • Collaborating and coordinates services with state agencies, the school system, primary care, and other service providers on the behalf of the youth and family and as indicated on the ICP.
  • Developing with the CPT a transition plan when the youth has made substantial progress towards achieving the goals of the ICP.
  • Obtaining insurance authorizations in accordance with the regulations of each Managed Care Entity.
  • Maintaining service productivity and/or caseload expectations as established by the program.
  • Managing flexible schedule consistent with family and program needs.
  • Demonstrating cultural competence in working with families with a variety of different cultural backgrounds .
  • Demonstrating working knowledge of community resources.
  • Implementing the 10 Principles and 4 Phases of Wraparound.
  • Completing All paperwork is completed within established timeframes.
  • Progress notes are submitted by 5pm the day after services are delivered.
  • Creating ICPs that are youth and family-centered and specify the vision, goals, options and tasks to address needs identified by CPTs.
  • Ensuring initial Risk Management/Safety Plans are completed and signed at time of enrollment.
  • Ensuring Risk Management/Safety Plans are reviewed and updated as needed and always within program specified timelines when youth has contact with the Mobile Crisis Intervention Team, discharged from a 24-hour facility, when another safety issue arises, and at transition from ICC.
  • Risk Management/Safety Plans detail response plans for families to prevent crises, provide stability during crisis situations, and to prevent the need for out of home services.

The Intensive Care Coordination (ICC) program at Behavioral Health Network is a voluntary outreach service that uses 'wraparound' principles to guide the work for youth with serious emotional or behavioral health needs. Wraparound is simply the coordination of multiple services via a tailored plan to address the needs of the youth.

What are we looking for in a Care Coordinator II?

Qualifications/Requirements
  • Master's Degree in a Human Services-related field that is license-eligible
  • Must be CANS-MA certified.
  • Must have knowledge of mental illness/SED as manifested in children and adolescents as well as knowledge of treatment techniques.
  • Must have familiarity with internet, word processing, and email systems.
  • Experience in navigating any of the child/family-serving systems (preferred).
  • Knowledge of community resources (preferred).
