Our client is currently seeking an RN Case Manager in Orlando, Florida!

Job Title: Case Manager RN

Job Type: Contract

Hours: 8:00 AM - 5:00 PM (Monday - Friday)

Location: Orlando, Florida 32806

Rate: Starting at $40.00/hour

Job Responsibilities:

• Conduct outbound and receive inbound calls to assess members' current health status.
• Identify gaps or barriers in treatment plans.
• Provide patient education to assist with self-management.
• Make referrals to outside sources.
• Ensure a complete continuum of quality care through close communication with members via in-person or on-phone interaction.
• Support members with condition education, medication reviews, and connections to resources such as Home Health Aides or Meals on Wheels.

Required Qualifications:

• Current, unrestricted RN license in the state of employment.
• Proficiency in typing and navigating a Windows-based environment.
• BSN (Bachelor of Science in Nursing).
• Bilingual in Spanish.

Required Experience:

• 1+ years of experience in an acute care or direct care setting.
• Experience or exposure to discharge planning.
• Case management experience.
• Background in managed care.
