Job Title: Registered Nurse Care Coordinator/Case Manager (Genetics)

• Start Date: 10/01/2024
• End Date: 09/30/2025
• W2 contract (benefits are available for the contract)
• Location: Durham, NC 27701
• Work Hours: Business Hours: Monday to Friday, 08:00 - 16:30 ET (Closed on weekends and all Federal Holidays)
  

Job Description:

• Triage all incoming consults within two business days of receipt.
• Contact patients with negative results within three business days of receipt.
• Maintain a record with no more than one substantiated complaint from patients or staff.
• Update the patient tracker with relevant information accurately and with minimal errors.
• Ensure consults to other services are input and ready for signature.
• Follow all established medication policies and procedures; no sample medications to be provided to patients.
• Provide discharge education and follow-up instructions coordinated with the next care setting for all Hematology/Oncology clinical patients.

Qualifications:

• Registration: Must be a registered nurse (RN) with a current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth
• Education: Graduate of a school of professional nursing approved by the appropriate State agency and accredited by one of the recognized accrediting bodies at the time the program was completed.
• Certifications: All staff must be currently certified in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS), or equivalent.
